What causes sciatica?
Sciatica is a symptom, not a diagnosis - it means the sciatic nerve is being irritated or compressed somewhere along its path. Common sources include a lumbar disc herniation or bulge, degenerative disc changes or spinal stenosis, joint dysfunction in the low back, and piriformis or deep hip muscle tightness pressing on the nerve. Identifying which of those is driving your case is the first job of the exam, because the treatment differs.
Can sciatica be treated without surgery?
Many cases of sciatica respond well to conservative, non-surgical care. Spinal decompression can reduce disc-related pressure, chiropractic care and manual therapy address how the low back and hip move, and rehab rebuilds the core and hip strength that keeps the nerve out of trouble. Treatment recommendations always depend on your diagnosis and individual condition, and we refer out when a surgical or specialist consult is warranted.
How do I know if it's really sciatica?
True sciatic pain follows a nerve pattern - it travels from the low back or glute down the leg, often past the knee, and may bring numbness, tingling or weakness with it. Pain that stays local to the low back or hip is usually something else, such as joint or muscle involvement. Our orthopedic and neurological testing is designed to tell those apart before treatment starts.
Do I need an MRI before I come in?
No. We start with a thorough history, physical exam and neurological assessment, and order imaging only when it will change your plan. If you already have an MRI or x-ray, bring it and we'll review it with you. When advanced imaging is warranted, we coordinate it with local imaging centers so care isn't delayed.
How long until I feel better?
Many patients notice meaningful improvement within the first few visits, usually starting with the sharp radiating pain. Restoring mobility and rebuilding strength so it doesn't keep flaring takes a series of visits. Your timeline depends on the underlying cause, how long symptoms have been present and your daily demands - and we re-evaluate at set milestones rather than guessing.
